Field Accident Report User Guide
This article will guide you through using the Field Accident Report.
If you are on a mobile device tap the Accident Report icon. 
That will open the form:

Any field marked with an * is required. There are a few required fields that may not be applicable.
You can enter anything but must enter something.
If at any time a box or text is too small, use two fingers to stretch (enlarge) the section of the screen.
Select the type of report:

Entry boxes will open up later in the form. Complete all the open boxes
for vehicle(s) property and people involved.
Set the date and time of the accident:
Set the time to the closest available.


Describe the location as best as you can:

Enter the exact address:
If the accident is not at a business, enter the address of the closest building.
If there are none available, you must enter something to proceed.
Please keep in mind that this information will be displayed on a report to Management.

Enter your name, Driver's License number and your supervisor's name.

Enter the Truck/Trailer information.
You may enter up to three (3) by clicking on the Add button in the lower left.

If other vehicles are involved, select Yes and enter the information

You may enter multiple vehicles by clicking on the Add link at the bottom left

Enter information about the damaged property.
You may enter multiple contact by clicking on the Add button at the bottom left.

Describe the damage:

Take pictures of any damage to vehicles or property.
DO NOT TAKE PICTURES OF PEOPLE.
If you are using a mobile device (tablet/phone) make sure to use the
"Choose Files to upload" option. That will give you the drop-down to select
your front facing camera or if you have already taken pictures, you can select
them from the Photo Library.


Enter information about other people that were involved.
Additional boxes will open as you may Yes selections.
Under Injuries and Witness, you may add multiples by clicking the Add button at the lower left.


The final section if for your statement.
Driver Statement is your chance to describe what happened.
After that type your name and sign (use your finger or a tablet pen)
and date the form. The last area is to allow you to draw a description of the accident.
There is a final Checklist to help you verify that you have completed everything.
If you are satisfied with your entry, Click or tap Submit.
The report will be sent to supervisors, management and HR.
